Family Peer Support & Services

Participants must be Medicaid eligible/enrolled
Anyone can complete a referral
Prior to services, a “Reference” is required from a Licensed Practitioner of the Healing Arts validating Medical Necessity
Strength-based goal and plan driven services designed to support parents
Provided by trained and credentialed Family Peer Advocates
Provided in individual and group settings in the home/community
